I, the undersigned, am the parent/legal guardian of the minor participant named above, or if I am the adult participant, I am over the age of 18 and fully capable of signing this Waiver and Release of Liability. I hereby acknowledge and understand that participating in the Silverbackz Basketball (SBA) AAU basketball program involves inherent risks, including but not limited to personal injury, property damage, or even death.

I voluntarily agree to release and hold harmless Silverbackz Basketball (SBA), its directors, officers, employees, volunteers, agents, and any associated persons or organizations, as well as the city hosting the event, from any and all claims, liabilities, causes of action, or damages arising from any injury, including death, that may occur during participation in any basketball practice, game, or event associated with SBA, whether caused by negligence or otherwise.

Acknowledgment of Risk:

I understand that basketball is a contact sport and that injuries are a common occurrence. I acknowledge that I have been informed of the potential risks of participation, and I voluntarily choose to participate or allow my child to participate. I understand that SBA will take reasonable precautions to ensure safety; however, I also acknowledge that no precaution can eliminate the risk of injury, and I accept these risks on behalf of myself and my child.

Medical Treatment Authorization:

In the event of an emergency where medical treatment is required, I hereby give consent to SBA to seek and authorize any necessary medical treatment for the participant. I understand that I am responsible for any medical expenses incurred as a result of such treatment.

Indemnification:

I agree to indemnify and hold harmless Silverbackz Basketball (SBA), the city, their directors, employees, agents, volunteers, and all other associated parties from any claims, lawsuits, damages, or expenses arising out of any injury or damage caused by the participant’s actions, inactions, or negligence during participation in SBA's activities.

Severability:

If any part of this waiver is found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.
