POLICY AND AGREEMENT

SILVERBACKZ CTA WEIGHT TRAINING PROGRAM

We are excited to continue building stronger, faster, and more disciplined athletes through our Silverbackz CTA Weight Training Class.  To maximize results and accountability, we are asking all athletes to commit to a 2-month training period. This commitment allows us to properly develop strength, consistency, and performance at a high level.


Program Expectations:  2-Month Commitment Required

Athletes must stay consistent and locked in throughout the full training period

Focus on strength, conditioning, and overall athletic development


Payment Policy:  Payments are due every Monday BEFORE workouts begin

No athlete will be allowed to participate until payment is received

This program is built for athletes who are serious about getting better. We are pushing for growth, discipline, and results.

WEIGHT TRAINING AND AGILITES

 We emphasize a “God First” mindset, accountability, and leadership, ensuring every athlete grows both on and off the court. Through intense weight training workouts, and competitive development, CTA prepares athletes to dominate in-season play and excel in life.  High-level development program designed to transform athletes from May through September through a combination of weight training, agility work, and basketball skill development. This program is built to help athletes become stronger, faster, quicker, and more explosive while sharpening their overall game performance. 

BY FILLING REGISTRATION FORM OUT BELOW, YOU AGREE WITH THE PROGRAM PARTICIPATION WAIVER BY TYPING YOUR NAME.

PROGRAM PARTICIPATION WAIVER
I, the undersigned parent/guardian, understand that participation in CTA SPORTZ Weight Training, operated by Silverbackz Athletics Association and/or Coach Thomas Academy, involves physical activity, strength training, conditioning, agility work, athletic drills, and other strenuous activities that may result in injury.


I acknowledge and fully understand that participation may involve risks including, but not limited to:
• Muscle strains or pulls
• Sprains and fractures
• Heat exhaustion or dehydration
• Slips, falls, or collisions
• Equipment-related injuries
• Serious bodily injury
• Illness or unforeseen medical conditions or death
By signing this agreement, I voluntarily accept and assume all risks associated with participation in the program.


RELEASE OF LIABILITY
I hereby release, waive, discharge, and covenant not to sue:
• The Oaks School
• Silverbackz Athletics Association
• Coach Thomas Academy (CTA)
• Coaches, trainers, volunteers, staff, officers, board members, and representatives
• Facility owners and operators
• Transportation providers associated with the program
from any and all liability, claims, demands, actions, damages, costs, or expenses arising out of or related to any loss, injury, illness, accident, or damages that may occur during participation in training sessions, transportation, camps, events, or activities connected to the program.


MEDICAL AUTHORIZATION
In the event of an emergency, I authorize program staff to obtain emergency medical treatment for my child if I cannot be reached immediately.
I understand that I am financially responsible for any medical treatment or emergency services required.


PARENT/GUARDIAN RESPONSIBILITIES
I understand and agree that:
• My child must follow all safety instructions and staff directions.
• Proper athletic shoes and workout attire are required.
• Hydration is strongly encouraged.
• No phones or headphones are allowed during workouts for safety purposes.
• Failure to follow rules may result in removal from workouts without refund.


TRANSPORTATION RELEASE
If transportation services are utilized, I understand transportation is provided as a convenience and I release all organizations, drivers, and representatives from liability related to transportation participation, except in cases of gross negligence or intentional misconduct.


PHOTO & MEDIA RELEASE
I authorize Silverbackz Athletics Association, CTA Sportz, and affiliated organizations to use photographs, videos, social media content, livestream footage, or promotional materials containing my child’s image for marketing, recruiting, branding, and promotional purposes without compensation.


ACKNOWLEDGEMENT
I certify that I have read this Release of Liability and Waiver Agreement fully and understand its contents. I understand that by signing this document, I am giving up substantial legal rights on behalf of myself and my child.
